We understand that health disparities are deeply rooted in social determinants such as education, economic stability, access to quality healthcare, and safe living environments. Addressing these factors is crucial in leveling the playing field and ensuring everyone has the opportunity to achieve their best health.
It's essential to meet people where they are and communicate in a way that resonates with them. This means using language that is accessible and relatable, so everyone can grasp the importance of understanding their health metrics, like blood pressure, cholesterol levels, and blood sugar. Knowing these "numbers" can empower individuals to take proactive steps in managing their health, preventing chronic diseases, and improving their quality of life. By speaking their language, we bridge the gap between healthcare knowledge and real-life application, making a tangible difference in the communities we serve.

The death rate for African Americans decreased 25% from 1999 to 2015. African Americans ages 18-49 are 2 times as likely to die from heart disease than whites. African Americans ages 35-64 years are 50% more likely to have high blood pressure than whites.
Tell a friend to tell a friend! Has your Bae had their Yearly Physical? Did they get their "tune-up"? Help make the appointment and take them to the doctor! Most preventative services are fully covered by your health insurance.
Young African Americans are living with diseases more common at older ages. Know your numbers. Make an appointment to visit your Primary Care Physician today.

Myth: I "know" if I have high blood pressure, and I just need to drink some garlic water. Wrong! Get the facts. High Blood pressure is like the engine in your car not getting the proper fluids!
Myth: I can only get "Suga Diabetes" if I eat a lot of sweet foods. Wrong! "Diabetes may be hereditary or "run in the family".
Myth: Only "older people" have strokes. Wrong! Period! African Americans are more likely to die at early ages from all of these causes. End of story!

We will guide you through the process of scheduling with your existing physician or help you find a new PCP.
What's a Wellness Exam? - A wellness checkup is a proactive approach to staying healthy. It's all about taking steps to prevent health issues rather than waiting for them to happen. Instead of only going to the doctor when you're sick, you schedule regular checkups to ensure you're still in good shape or to catch any potential problems early on. These checkups usually happen once a year and are separate from visits for specific illnesses or injuries.
WE ARE NOT DR's! But, as Community Health Workers our goals are to find and use proven programs to reduce disparities and barriers with goals to create opportunities for health! We work with other sectors, such as faith and community organizations, education leaders, small businesses, transportation providers, and housing providers, to create social and economic conditions that promote health In the Hood. The list is always growing!
We want to link more people to doctors, nurses, or community health centers to encourage regular and follow-up medical visits. Develop and provide trainings for healthcare professionals to understand cultural differences in how patients interact with providers and the healthcare system.
Why? Because "Health is the new Wealth", and someone cares about you.
